The label on record is marked "DT-2458" and the dead wax is marked "DT-2458" and cover is marked on bottom right corner "RIAA-5".

CONDITION - The LP is in VG to VG+ shape; there is some blemishing on both sides; one side worse than other, and some very light scratching on one side; albeit still sounds good.  The COVER is in good shape with NO rips, tears, holes or cuts; NOT from a discount bin; only real blemish is name neatly written on back side top right corner.  And neatly written on label too of record.  The cover is plain white.  Please see pictures.
