RCA Victor LPM-2011 Please see the 20 pictures that all enlarge to an enormous size.
In the original Paper Sleeve that is coming apart at the sides. I graded the record as Ex, and the cover graded as VG+, but look at the pictures and make up your own mind as to the grading.
