LADY MADONNA/THE INNER LIGHT (Capitol #2138 1st pressing w/PICTURE SLEEVE & FAN CLUB INSERT, 1968): Clean M- fan club insert; VG+ picture sleeve & 45.
 

The clean M- fan club insert wasn't included with every copy of Lady Madonna issued in 1968.  This insert is much scarcer than its current book value of $50 would indicate.  Our package includes a clean picture sleeve without writing, splits, or tears; it's got very little of the ring wear that's found on almost every example of this sleeve.  The single is a clean VG+ with pristine labels; another seller might grade it M-, but you know how picky we are.   
This was The Beatles' last Capitol single, with Paul channeling Fats Domino.  On the flip, George muses "...the farther one travels, the less one knows...".

Catalog value in this condition would be over $150...
