VG+ (very good plus)
Record has been played more times, it has more marks and surface noise, it doesn't skip; cover has a little fade in colour, and has more damage on the corners or edges.
VG (very good)
Record has been played many times, may be more surface noise, but it shouldn't skip while playing; cover has more cuts on corners and edges and more fading in colours.
G (good)
Record will have many marks of use, significant surface noise and scratches. Cover will have scuffing of edges, spine splits, tape, writing, ring wear, etc.
