This LP has what many Blue Note collectors look for in the dead wax between the runoff grooves:
The VAN GELDER machine stamp and the Plastylite pressing plant ear symbol (actually a "P") on both sides.
The record company address on the back cover is 43 West 61st St, New York 23. The address on the label is NEW YORK USA

I'd rate the record as VG+ (very good) according to the UK Record Collector grading guide, as follows:
VERY GOOD: The record has obviously been played many times, but displays no major deterioration in sound quality, despite noticeable surface marks and the occasional light scratch.

I have listened to both sides from beginning to end.
There are some pops & clicks, and surface scratches and marks, but the record plays fine.
In my personal opinion it sounds better than it looks.

The cover has visible creasing round the bottom right front corner (about an inch each way) and minor creasing round the other corners.
It also has a roughly 4 inch long open split along the bottom.
The back cover has a few stains and brown discolouring along the edges (mostly along the open side, see pic), there are some minor splits/paper damage along the opening edge, and a 4 inch crease close to the top right corner below the "Blue Note 4119" catalog number. That crease looks like it's only on the slick, not the cardboard cover.

Going by the Record Collector grading system, the cover might be considered a G (good).

The original record company inner sleeve is included but it's in poor condition.
The LP is stored in a generic paper inner sleeve.

I do not add anything for packing! The postage is what the local post office charges for a solid LP sized cardboard package by registered mail. The record will be removed from the cover to avoid damage during delivery.
This is a rare item so I’d be glad to send it insured. That would add to the total cost, but that would be the buyer’s choice.
In any case, do not pay before you get the invoice.
