I think this is a reissue not sure.  Couldn't find a lot on this label but it isn't the Green label
This record plays very well.   There were a couple  (literally very few) of very light crackles which I'm sure would go away with a proper wipe down (I didn't want to do anything to this record).   
The sleeve is an odd sort.  It's lighter weight than most and so it has a bit of a bend.  Especially on the lower left of the back there is a crease ... Think triangle about 1" up to 3" in (right on the Brain logo see pics).  The main problem is that this sleeve has a think plastic laminate that is peeling mostly on the back.
The inner sleeve is plain white with 4912 00 1533/W5/79 stamped on the lower right corner.  The inner sleeve is in nice shape.
