Decca SXL 2214 - 1960 Original UK stereo pressing.
Wide-band grooved label ED1. 'Original recording by' at 10 o'clock.
Scalloped back cover.
Cover : NM c/w original fss inner sleeve in nm condition
Record : EX (The record needs a professional clean but as far as I can see is very nearly mint. It has no real scratches to speak of but has half a dozen small indentations in the surface which can be seen on both sides. I have seen similar indentations on other Decca records of a similar age and in my experience they have very little or no effect on the audio quality)

Mint - only if record is sealed and never played regardless of visual
Near mint - clear shiny vinyl, looks unplayed - may have the odd miniscule mark but rarely
Excellent - clear shiny vinyl, maybe one or two small surface / needle marks
Very good - a few light surface marks
Fair - no deep scratches, quite a lot of surface marks, slightly dulled
