Decca   Release Date 21-11-69 As far as I can tell, this is an original first pressing and I definitely acquired the record around that time, although I'm not sure how, since I notice it is a Demonstration Sample and therefore theoretically not for sale.  Still, it was the '60's :)
Adhesive label on A side has T H Enterprises written on it, and then crossed out in order to put my mark (TT) and my best friend's name on (presumably it was a shared acquisition).  According to the label, the promotion company was Tony Hall Enterprises - I assume this was originally a copy for them.
Runout markings :
Side A Matrix stamped    XDR - 46195 - T1 - IC          B       1       JT
Side B Matrix stamped    XDR - 46196 - T1 - 1C         B       1       JT
Vinyl - NM - couple of small yellowish marks on run-out Side A.  Track ends very abruptly - that's the way it was recorded.  Few split-second crackles Side B, but really hard to find fault with play quality.
Labels - NM - clean, bright and unmarked with just a few spindle marks.  Play time and release date printed on label.  Adhesive label with details as noted above.
Sleeve - NM - slight tanning and minimal creasing.  No tears or marks.  Seams secure.
Like many of the singles I am now selling, this record has belonged to me (almost) from new.  Well cared for - stored always in sleeve and in record rack in cupboard, away from dust and dirt.  Has been played at most 5/6 times in last 40 years, including 3 times while listing to check quality for description (and to relive old memories !)  
Please ask if there's anything else you'd like to know, or if you'd like to see more photos.  Have tried to capture as much detail  as possible in the pictures, as these are the best description.  Didn't realise importance of matrix numbers etc until after photos were taken so if these don't show in the photos and you want to see for yourself, just ask :)
I have played the record through on both sides prior to listing and graded it according to a system which seems to be standard for many sellers :
MINT  The record itself is in brand new condition with no surface marks or deterioration in sound quality. The cover and any extra items such as the lyric sheet, booklet or poster are in perfect condition. 
EXCELLENT  The record shows some signs of having been played, but there is very little lessening in sound quality. The cover and packaging might have slight wear and/or creasing.
VERY GOOD  The record has obviously been played many times, but displays no deterioration in sound quality, despite noticeable surface marks and the occasional light scratch. Normal wear and tear on the cover or extra items, without any major defects, is acceptable.
GOOD  The record has been played so much that the sound quality has noticeably deteriorated, perhaps with some distortion and mild scratches. The cover and contents suffer from folding, scuffing of edges, spine splits, discoloration, etc.
