This is a description of the sleeve that came with the record originally (although it may not be the actual original sleeve). You will therefore, end up with two sleeves, the original and the replacement.

Minimal / negligeable signs of spindle markings. Label in super condition.
Disc Grading
Excellent ++
A lovely disc. Has responded very well to cleaning.
Grading Hierarchy
I am sure most collectors recognise the hierarchy of grading that seems to be used pretty universally. To be clear, I use the following with adjustments + or – accordingly.
MintRecord has not been played. Shows no signs of use at all. Outstanding condition of both record, sleeve and cover.
Nr Mint
Slightest signs of wear to cover, sleeve and no marks to disc. No clicks or pops.
Excellent
Slightly more wear signs overall. Cover will be excellent, label will have minimal spindal markings. May have some non sounding markings. Still an excellent disc with nothing significantly detrimental to performance listenability.
Good
More signs of wear than "excellent" such as fading to cover front an back and / or inner sleeve. Non sounding marks and may have one or two ticks, clicks or pops which I will be detailed.
Fair
Will not appear on eBay listings
Poor Will not appear on eBay listings
Please note. I do NOT use a single overall descriptor for the record as this can be misleading. I prefer to describe each element individually. If there is greater surface noise than I consider should occur on an LP of 40 / 50 years old I will detail it.
Records are graded on a Linn Sondek with Ekos Arm and Troika MC cartridge.
