Visually graded condition of disc: Near Mint (never played - spotless copy) 
 
Condition of Sleeve:  Sleeve is a bit rough from sloppy shipping from Columbia to my radio station. Has small creases on all sides except opening. Tiny split trying to form in middle of spine but hasn't broken through. Gold promo stamp on back. (darkness on bottom edge is a shadow)

Some carefully, others were not.
All of my gradings are strictly visual, and more often than not, quite generous.

MY GRADING SCALE
(very similar to the Goldmine standard)

NM (Near Mint): Record will show no serious scuffs or scratches in the playing area. May have minor scuffs on edges from decades of storage, or a small scuff, scratch or paper scuff on playing area.
It's near mint, not mint (which would be 'sealed' to me). There should be no audible distractions.
M-/NM- (Near Mint minus): A scuff or two or light scratch or two on either or both sides that may be heard.
VG+: Scuffs & some minor scratches in the playing area. Some will most likely be heard.
VG: Many scuffs or scratches that will be heard but record should still play fine.
