The catalog number on side 2 label is SD 19126 (ST-A-681462-SP), matrix in run out groove is ST A 681462 M 1-3 [SRC logo]

Vinyl is VG++/VG+ with some light marks - nothing deep or able to be felt.
Cover is NM/VG++ with very light wear. Any marks are on shrink wrap and not cover.
