The record is VG+ ~ The record has a great gleam with a couple very minor markings which should not effect the audio quality whatsoever. 
The sleeve is VG ~ There is some ring wear to each side, the cover is creased and slightly bent, the corners are slightly dinged, the edges have some wear and are split and were at some point repaired with tape. (see photos)
