Unfortunately I do not own the 7"single or picture sleeve anymore, I sold them a long time ago but I found this in amongst a bunch of old gig flyers when I was having a clear out.
I am pretty sure this is an Original, but I can make no promises. I found nothing to indicate that there were any bootleg versions of this insert on www.misfitscentral.com
