Please look at pictures to determine grading for yourself.  First picture is a stock photo, the rest are actual pictures of the item you will receive. Small center hole outlines in pic sleeve front and back(see pics).  Top right corner has what looks like the remainder of a price sticker glue removed long ago (see pics).  Record has never been on a turntable. Any "ghost" images on the vinyl are NOT damage, just artifacts from the light and flash and/or dust (im not the best photographer). "Made in England" sticker on back cover. The first time it has ever been out of the sleeve was to take these pics for this sale.  I bought this off a promo display on the counter at waxtree records in orlando florida 1991 and then put it away in my collection.
Famous (infamous) swimming baby on back cover promoting the Nevermind album as well as promo list for the upcoming British tour dates for that year.
