Mint (M)   Absolutely perfect in every way.
 
Near Mint (NM or M-)   A nearly perfect record.

Excellent (EX)   A record that shows some signs that it was played and otherwise handled by a previous owner who took very good care of it.
 
Very Good Plus Plus VG++ A record show some signs that it was played and otherwise handled by a previous owner who took good care of it, but the record surfaces or sleeve may show some more  signs of wear.
 
Very Good Plus (VG+) Still a good record but it may have scuffs or light scratches that hardly affect one's listening experiences. Picture sleeves and LP inner sleeves will have some edge and/or ring wear, lightly turned up corners, or a seam split. 

Very Good (VG)  Many of the defects found in a VG+ record will be more pronounced in a VG disc. May have some audible scratches, scuffs, surface noise and visible groove wear....still a nice play copy
