These were given as gifts at Jack White's "Rise and Fall of Paramount Records" NYPL appearance on November 19, 2013, with a panel to discuss and promote the release of the Paramount box set Third Man has released. See the pics attached of the record, you get exactly this record, in this sleeve. Also included is a letterpress card from Isle of Printing (available only at this appearance) and a promotional card for the Cabinet of Wonder.
These 78s were never sealed and were handed out just as you see it in the pictures. Since it is unplayed, I am grading it MINT. The sleeves are old stock which were bought and used by Third Man and Revenant to house these 78s. They are all different and unique!

GRADING
I grade vinyl very conservatively. I don't just sell records, I buy them for myself as well. The only records I will grade Mint are sealed records, and even then will note cover condition. I do not rate records Near Mint. The next-highest rating for me is Excellent.
Mint = sealed
EX = like new condition, little to no wear visible
EX- = may have a sleeve mark or two that does not make noise, or slight sign of handling, but still plays excellent
VG+ = no major flaws, looks good and plays great, occasional minor pop or click, used but not abused
VG = may have some marks that play but no skipping; may have some pops and clicks. Still a solid record to listen to.
