No label, but the Hot Wacks book calls this Berkeley 2811/2812; recorded at the Forum in Inglewood, CA, Jan 18, 1973.
Blue & white gatefold cover, actually a single folded sheet with inside flaps. Front is supposed to be a photo from the actual concert, and the back is cartoon panels illustrating the song titles, by William Stout. Decent quality audience recording. Appears to be a scarce label and cover for this.
