This is an original vinyl issue, from 1974 "Transparency" by Icelandic progressive/psychodelic rock group Roof Tops. It 's the first and only album released by the the band.
It's almost impossible to find this record in any condition ! ! !

LEBEL/SLEEVE/INSERT
NM Crisp and clean with no creases/ bends or discoloration.
NM- Crisp and clean, with maybe one or two small creases or discoloration, common with age.
EX+ Clean but maybe minor creases or discoloration.
EX- Clean, maybe a few creases/bends. Some discoloration and/or lamination lift.
VG+ Few creases or bends. Some discoloration, edges might be a little tatty, even minor tears, and lamination lifts.
VG Many creases or bends. Lamination lifts.
