This is the black label w/colorband issue of the LP, not the green label. The "ST 8-2047" in the upper right corner of the cover is the identifier for this. 
There are three "///" in the upper left corner of the front cover that flows over to the back cover. There is a "16" next to the RIAA symbol on the back cover.
The two upper corners have a "push." I can see no breaks in the shrinkwrap. There is some "foxing" noticeable on the back cover. I'm assuming that this is from miniscule breaks in the shrinkwrap somewhere. 
Very hard to find this LP sealed!
