Decca Records stereo three disc set, SXL 2054-5-6, first grooved ED1 wideband label discs have "Original Recording By..." at upper left, made in England.  Black box.  Stampers are 7G 5K 9G 9G 5E 8G.  Includes the full size libretto. 

Puccini, Madama Butterfly, complete opera, with Renata Tebaldi and Carlo Bergonzi.  Tullio Serafin conducts the Chorus and Orchestra of the Accademia de Santa Cecilia.  See pics for complete cast.
