Soundgarden ULTRAMEGA OK SST 201 SST Records 1988 018861020112
This record is still sealed, the record unplayed, and has not been removed or opened at all since I bought it in 1995 from a dealer.  The corners are a little scuffed from storage, as shown in the attached pictures, but is otherwise in great condition. 
As it is unopened and sealed, I've no idea as to the colour of the vinyl and have no way of checking so if anyone knows of any indicators please let me know.
