I have visually looked / tested the record and it plays fine - Sounds great.  
There are surface scratches and noise that do not affect the listening pleasure.
There is a slight warp.
The record was cleaned
Comes with a Generic Sleeve
 Shipped in a box made for 45 RPM Records
The record was played on a Jensen JTA - 230 record player.
I referenced the Goldmine Grading Guide for grading the Vinyl record.
