One of the best punk records to have ever come from Canada and a true rarity.  The "Tomorrow Belongs to Us 4 track EP" by The Forgotten Rebels on S & M Records.

Sleeve and Vinyl are both excellent and it actually looks unplayed but I know that isn't true as I've given it a few spins!
