Original 45 on  Syndrome Records, Canadian Pressing. 

Syndrome 1001, from 1965 (Label later changed to 'New' Syndrome)

Not many made, and few in existance today, especially in good shape, very difficult to find.

Disc: looks close to Near Mint, except for a small round edge mark on both sides
that makes some light noise as it rotates near the beginning of the song
