World Youth Records, 5-9 Kitangasadori, Higashi-Ku, Osaka, Japan. 3 inserts: 1 12x12" page of stills from Andy Warhol's movie The Velvet Underground And Nico (aka A Symphony of Sound; 4 sided purple A4 paper typed article of The View from the Bandstand by Lou Reed.
This is number 0800/1000. Released in 2005.
