Includes: Original 12" vinyl record, original outer cardboard sleeve, original generic white inner plain paper sleeve (for the LP), a clear outer protective sleeve that I added (to protect the outer LP cardboard sleeve)
I purchased this LP new and I've been the only owner. This copy did not come with a printed insert, but from what I understand some pressings did include one. This one came with a generic white inner sleeve for the LP.
I'm not a professional LP seller so I'll do my best to explain the condition below. I also provided the maximum number of photos allowed to show you as many views as possible. 
Vinyl LP: Overall the record looks and sounds great and has only been played a couple of times. There are some light hairline blemishes visible when I hold it up to the light. It came out of the packaging that way and from what I understand that can often be caused by the record rubbing against the paper sleeve (but I'm not certain if that's the case here). 
Outer LP sleeve: The integrity of the outer sleeve is strong and has some incidental shelf wear (especially on the corners and edges) and no seam splits. There's a bend/crease in the upper left corner and another smaller bend/crease in the lower right corner (see photos). 
Inner sleeve/inserts: The white insert is in good condition with only minor incidental wear from gentle use.
