Label address reads : Blue Note Records 161 Lexington Ave NYC.  RVG hand-etched on deadwax!!

Record: 

EX: LP is in excellent shape, a few very minor scuffs and 1-2 very faint scratches but that is all, record still maintains it's shine.  A solid EX rating!

Cover:

VG++ : Cover is in very good shape no tears or seam splits anywhere, cover looks a little discolored but nothing major.
