1982 re-pressing of 1000, Dischord 5/Limp 41, yellow labels.
This copy belonged to Jo Smitty, singer of the legendary hardcore/art noise band Mr. Epp who bought it from the band by mail-order in 1982.    I put the sticker on my saxophone and it's still there (see photo of me and Mark Arm in Epp circa 1983) 
This a rare chance to own one of the greatest hardcore punk records of all-time and it's a copy with a sought-after provenance as a bonus.  Wow!
