This is disc 00 in the Neil Young ArchivesPerformance Series, catalogue number 512563-1. 

This was a gift to me and has been in my personal collection. I don’t thinkit’s ever been played. Even though the plastic cover is ripped, I have kept it in the plastic cover stored with my other vinyl records. Everything is in very good condition. Only defects I see are a slight splittingon the bottom edge of the gatefold about 1” on front and 3” on back, one minorcorner crush (top right when viewed from front) and small slash on back bottom corner.
