Catalog numbers: V6-5008 on sleeve. B side is V-5008 MG-569 (Mono) A side is V6-5008 MG-568 (Stereo)
After much research on this album, I believe the owned album photoed, is this.  If you believe this is something different please let me know.  I am making my most educated guess on this, but there are very many versions of this album as you know.  I tried to give the most amount of pictures possible.
