During the manufacturing process, the stamper became damaged.
Consideration was given to sending new stampers from the UK to Japan. However, in the end no effort was made to replace it - and production was halted. This resulted in a very limited number of existing copies.
(It's estimated that only about 1,250 were pressed before the damage occurred and production was halted.)
