This is a Zonophone Coronation Puzzle Disc No. 598 made for the coronation of King George V and Queen Mary of the UK in 1911.  It comprises

A ‘puzzle plate' is a record that has more than one grove on each side. In this instance, there are three grooves per side; when the needle drops, any one of the three grooves could be picked up.
